SoFlo Community Health Foundation (SoFloCHF) is a South Florida nonprofit committed to removing the barriers that keep people from getting the healthcare they need. We work alongside trusted local partners to connect uninsured, low-income, and underserved youth to quality medical care — including sports physicals, cardiac screenings, primary care, and preventive health services.

Florida law now requires EKGs for high school student-athletes. Through our partnership with UltaMed, we provide 12-Lead EKGs for $10 and/or a full sports physical with EKG for $35 — making compliance affordable for every family.
